Once you have selected radio button list click the new button.
Next you will need to type a "Property Name" which is displayed on the order (for administrators) and then type a "Display Name" which is displayed to the customer.

Then you can decide if you would like the radio buttons to display in a vertical list that fills 1 column or in a row with several columns.
Lastly, you will now need to add the radio button list options by adding a new item per option. Click the new button to add items.
Now you will need to type in a display name of the option, a value for the order that will be received in your store, and complete the remaining options for this item, when finished click the ok button to be able to add additional items.
Once you have added all the items you would like click ok to save the radio button list.
